To refer a patient to InCare from your office we will need the following:

  • Referring Office Name & Insurance Reference
  • Relevant Patient Testing Results
  • Date of Patient’s Last Visit with Referring Physician’s Office

How Can I or My Family Member Start with InCare?

Before we can help, a referral must be made first by a primary care doctor (PCP).

Please have a PCP fax patient referrals to the location you would like to visit.
